How Short Term Disability Benefits Work
Apply for Temporary Disability Insurance benefits only if you have stopped working and your doctor has told you that you are unable to work because of your pregnancy.
You can apply for Temporary Disability Insurance online, by mail, or by fax. You will find instructions with the application. Read all instructions carefully before completing the application. If you submit the form by mail or fax, sign and date the form.
When you apply for Temporary Disability Insurance benefits, we need information from you and your health care provider. It is your responsibility to send us all of this information – including the Medical Provider section – online, by mail or by fax.
When you fill out your form online, you will be given a unique online identification number that you can give to your healthcare provider. They send us their parts online using this number. You will be asked to print a page (or save it as a PDF file) with your ID number on a special online form and instructions for your healthcare provider to follow. If you do not have access to a printer when you first submit your application, you have fourteen days to return and print the instructions once you have a printer. You can do this by clicking Access Claim Documents at the top of this page.
Short Term Disability Insurance
If you plan ahead, you can start your online application up to 60 days in advance. To avoid cancellation of your draft application: (1) verify your application by providing all information within 14 days of initiating the application; Then (2) confirm and submit your claim within 14 days of the start of your leave.
If you submit your application after the start of your leave, you have 30 days from the first day of leave to submit your application.
To make a valid claim for temporary disability insurance, you must have paid through your employment plan and meet the minimum gross income requirements (listed below). Temporary disability insurance is available to most New Jersey workers.
If your employer has temporary disability insurance, he/she is obligated to determine your wage contribution for you. If you think you are in the system, but have not made a contribution to the salary, you are encouraged to apply because there may be a mistake. If you have recently been unemployed and need temporary disability insurance, you may still be eligible for benefits and we recommend that you apply. Your employer is required to report your wages to the civil service after each calendar quarter. When assessing your complaint, we will take into account the income of the five quarters completed before your holiday week begins. The first four quarters of that period are called the academic year.
To qualify for temporary disability insurance in 2023, you must have worked 20 weeks and earned at least $260 per week, or earned a total of $13,000 in those four quarters (the base year).

2. Now we can determine your weekly benefit percentage. Applicants receive 85% of their average weekly earnings, up to a maximum weekly profit percentage established for that year. In 2023, the maximum weekly benefit will be $1,025 per week. In 2024, the maximum weekly benefit will be $1,055 per week.

Benefits are generally paid up to four weeks before the expected delivery date and up to six weeks after delivery (or up to eight weeks after cesarean delivery).
