A new version of a popular diabetes drug could be sold as a weight-loss drug in the United States, regulators said Friday.
The Food and Drug Administration has approved Wegovy, a higher dose of Novo Nordisk’s diabetes drug semaglutide, for long-term weight management.
In studies funded by the company, the weight of participants who used Wegovy dropped by an average of 15 percent, or about 34 kilograms (15.3 pounds). Participants lost weight steadily for 16 months before reaching a plateau. In the comparison group, which received sham injections, the average weight loss was about 2.5%, or slightly less than 6 kilograms.
“With current medications, you can get maybe 5 to 10 percent weight loss, sometimes even that,” said Dr. Harold Bays, medical director of the Louisville Metabolic and Atherosclerosis Research Center. Bays, who is also the chief scientific officer of the Association for Obesity Medicine, helped conduct research on the drug.
Powerful New Obesity Drug Poised To Upend Weight Loss Care
Even a 5 percent weight loss can lead to health benefits, such as better energy, blood pressure, blood sugar and cholesterol levels, but that amount often doesn’t satisfy patients focused on weight loss, Bays said.
Bays said Wegovy appeared to be safer than previous obesity drugs that had “come under fire” for safety concerns. The most common side effects of Wegovy are gastrointestinal problems, including nausea, diarrhea, and vomiting. This usually goes away, but causes about 5% of study participants to stop using it.
The drug has a potential risk for a certain type of thyroid tumor, so it should not be taken by people with a personal or family history of thyroid tumors and certain endocrine tumors. Wegovy is also at risk of depression and pancreatitis.
Wegovy (pronounced wee-GOH’-vee) is a synthetic version of a gut hormone that suppresses appetite. Patients inject it weekly under the skin. Like other weight loss treatments, it should be used in conjunction with exercise, a healthy diet, and other steps such as keeping a food diary.
Swedish Regulator Asks Doctors Not To Prescribe Diabetes Drugs For Weight Loss
The Danish company did not disclose Wegovy’s price, but said it would be similar to Saxenda, an 11-year-old weight-loss drug that is injected daily and now typically costs more than $1,300 a month without insurance.
Dr. Archana Sudhu, director of the diabetes program at Houston Methodist Hospital, said Wegovy’s benefit “all depends on what the costs are going to be.” He said patients’ health insurance plans sometimes don’t cover weight-loss treatments, making expensive drugs prohibitive.
Sudhu, which is not affiliated with Novo Nordisk, plans to transfer patients suffering from obesity and type 2 diabetes to Wegovy. This makes patients feel full quickly and increases insulin secretion from the pancreas to control blood sugar, he said. Patients are then more likely to be motivated to exercise and eat healthier, he adds.
Wegovy is building on a trend where relatively new diabetes drug makers are testing them to treat other conditions commonly seen in people with diabetes. For example, the popular diabetes drugs Novo Nordisk Jardiance and Victoza are already licensed to reduce the risk of heart attack, stroke and death in heart patients.

Phylander Pannell, 49, of Largo, Maryland, joined a study of patients who tracked weight loss and then weight regain. He said that he received Wegovy, that he exercised several times a week and that he lost 65 kilos in 16 months.
“It helps my appetite and helps me get there faster,” Pannell said. “It set me on the right path.”
Shortly after the study ended and Wegovy was discontinued, he regained about half his weight. She has since lost most of it, started exercise classes and bought home exercise equipment. Dr. Greg Katz: The disappointing data on long-term dieting is the single best argument why I think the right way to think about obesity is that it’s actually an environmental disease. The modern environment, the way we eat, move and live, causes obesity.
Dr. Shreya Trivedi: Yeah, Greg. I like this idea of thinking of obesity as an environmental disease and everything fat in our society, I think it’s just a powerful frame of mind.

Dr. Greg Katz: That’s the best way I’ve found to think about it. You can retreat from the accusations of individual responsibility, food companies and farm subsidies and focus on the essentials. What can we do to help obese patients?
Dr. Clem Lee: Unfortunately, the medical history of obesity is a sad one. It’s full of side effects, ineffectiveness, and doesn’t really make a dent in the problem.
Dr. Shreya Trivedi: Yes. Many drugs that are proposed to help people lose weight turn out to be dangerous. Amphetamines, exogenous thyroid hormones, fenfluramine, ephedra, locaserin. The trade-off of weight loss in exchange for unexpected, catastrophic side effects such as pulmonary hypertension, increased cancer risk, and cardiac arrhythmias is simply not worth it.
