Breast milk is created by supply and demand. The sooner the baby will suck at the breast, express manually or with the help of a breast pump, the faster breast milk will be produced to meet this need. Accordingly, a larger total number of pumps per day will be more profitable to supply than a smaller number of pumps spaced apart. Frequency depends on why you are expressing breast milk and how much breast milk is needed – unnecessary over-expressing can contribute to engorgement (mastitis) and/or excess milk.

If the baby is not nursing, the mother can pump when convenient, trying to do as many pumping sessions as possible to mimic the normal feeding pattern of a newborn (eight to ten feedings per day). However, if your baby is also breastfeeding, it can be difficult to determine when is the best time to pump, or even find a time to pump. Strategies include:

When pumping, the goal is to pump long enough to effectively remove milk from all parts of the breast and stimulate the mammary glands to increase production. A nursing breast is never empty, so in practice this means removing or “expressing” most of the available milk. The ideal pumping time to achieve this will vary from woman to woman depending on:

A study of mothers of preterm infants found that double pumping with massage (hand pumping; see below) lasted an average of 25 minutes, with intervals ranging from 15 minutes for some mothers to 45 minutes for others.3 Lactation consultant Nancy Morbacher recommends starting pumping. for 20 minutes and beyond to adjust the pumping time according to the flow of breast milk: simply pump for a longer or shorter period to get all the milk out (Mohrbacher. 2020. p. 490).

The more often you pump your breasts, the more milk will be released. If you haven’t pumped eight or more times in 24 hours, try increasing the number of puffs. If you are close to your baby, try to pump as often as your baby normally breastfeeds.

Massage or manual pumping is said to significantly increase the amount of milk pumped. Manual pumping involves light massage and squeezing of the breast with hands before and during pumping. One study found that the amount of breast milk per day increased by almost 50% per day when it was expressed by hand5, and the caloric value of the expressed milk also increased6. Jane Morton, MD, of the Stanford School of Medicine, has developed a helpful video called “How to Express Breast Milk” (see 5:15), and “Exclusive Breast Pumping” also explains the technique.

Double pumping (expressing both breasts at the same time) saves time and can help produce more milk1011, although it is not necessary for good results12. If your baby isn’t around, pictures of your baby or an item of clothing that smells like your baby can help keep the oxytocin flowing.

One study of mothers who expressed milk for premature babies found that warming one breast with a warm washcloth 20 minutes before expressing produced significantly more milk than an unwarmed breast.

Helpful Ways To Increase Breast Milk Supply

Lubricants inside the pump flange and/or on the breast can make pumping more comfortable by reducing friction. Mothers used oils (such as coconut or olive), breast milk, or lanolin as lubricants. Marasco and West state that with the correct size pump flange, lubrication is not necessary (Marasco and West. 2020 p. 196) – it is important to make sure the nipple is in the center of the flange and that you are using the pump correctly. (see below), but some fat may be beneficial for some mothers.

Studies have shown that mothers who listened to recordings, turned on relaxing music and looked at pictures of their baby while expressing were able to express more milk.19202122

Expressing with these pre-pumping routines can help some mothers relax and let the milk come out. This can be important for mothers who have difficulty expressing milk with a pump. Nancy Mohrbacher recommends wrapping your arms around your shoulders for warmth, giving yourself a gentle massage, and spending five minutes doing relaxation techniques before pumping. Some mothers find that looking at milk collection bottles to see how much milk they are getting at each session can be quite stressful. Covering the breast pump with a sock or glove can reduce the amount of milk you collect, and if it helps you feel more relaxed while pumping, you’ll be able to pump more milk as a result.
